Question about physicals

I need to have physicals done for all the kids. Since we are using a civilian doctor they want me to provide a sheet. Well I have no way of getting one but thought I could scan an old DOD one and white out the info??? Are they all pretty much the same regardless of where you are?? ANyone know?

Are they online? I know the VA is putting most of it's forms online which makes things a lot easier.

Awww crap...Heather I need you!!!!

The form I have is the DA form 5223-R which is for the CDS... grrr not a physical. Wonder if it would still work??

Thanks Lenora. I just thought about that too but I have no idea what the form is I need.

This is what I found, I hope it helps:
PHASE 3 Medical Screening (Mannheim Health Clinic, Fridays 1300-1600 hrs)

POC: EDIS contact the front desk at 385-3133

Mannheim Health Clinic - contact Central Appointments 380-4095/4096



Step 1 - Sponsor brings DA Form 5888-R, Medical Records, completed DA Form 7246-R, and family

members who require screening.

Step 2 - Provider will conduct examination and complete the following forms:

-Part B, DA Form 5888-R

-Preschool Developmental Questionnaire for children 72 months and younger

-DA Form 5862 if enrollment is warranted

-Addendum(s) to DD Form 2792 (for Asthma and Mental Health Patients only) as needed

-Refer children under 3 to EDIS if warranted.

Step 3 – Case Coordinator will:

-Review forms to ensure all information is complete.

-Collect DA Form 7246-R (provide sponsor with copy, if requested)

-Collect Medical Record and DA Forms 5862 and addendums to DD 2792 (provide sponsor with

copy)

-Refer sponsor to DODDs case study committee to complete DA Form 5291(for children with

special education needs)

Step 4 - Send completed forms to Special Needs Advisor at Heidelberg MEDDAC.

Step 5 – Put copy of 5888-R and front sheet of 5862 in EFMP box.

None of those were right either. Court is supposed to go to the clinic there and get the form for me.

Meagan, that is probobaly the best bet. Than you know it's the right paperwork. Have him fax it to you, or straight to the Doctor's office so you don't have to wait on it.

He said there was some way to make it into an adobe acrobat PDF (think that is the right terminology) and I am just going to print it out for the doctor.
