Babysitter application

My friend and I are getting ready to put an ad in the HOA newsletter to find some babysitters to cover the days the kids are out of school (aimed at high school/college kids). I'm thinking about putting together a small application that we can hang on to with the sitter's info and was just looking for some suggestions.

Off the top of my head, I'm thinking:

Name
Address
Phone/cell/email
Parents Names

School/Grade

References

CPR certified?

Days/times available


Am I missing anything else important??

What about a space to to summarize thier experience?

What about if they've taken the baby sitting class through the local school?

How about their asking price per hour / per child?
